• Medication review, initiation, adjustments and maximization of recommended medications and device therapies
• Educational programs, diet assessment and exercise program through Cardiac Rehab, if applicable
• Experienced heart failure nurses to educate patients and families as well as monitor patient status
• Individualized multidisciplinary disease management plans to get you back on track
• Comprehensive diagnostic data collection and analysis from implanted defibrillators
• Laboratory and diagnostic services and monitoring
• Quarterly progress reports to your cardiologist or referring physician
• IV diuretics, if the need arises
