Life is full of decisions, but one of the most important decisions you can make for you and your family is to determine your advance directives. Informational sessions will be offered on advance directives featuring legal and medical experts. A team will be available to help you complete your advance directives or answer any questions. What is an advance directive?
10 – 11 a.m. - Matthew Cope, M.D.
2 – 3 p.m. - Elpidia Balbastro, M.D.
If you experience a serious illness, advance directives are written documents that state your choices for health care, or name someone to make those choices, if you become unable to make decisions.
Advance directives provide an opportunity to:
• Make legal, valid decisions about your future medical treatment.
• Plan ahead to assist your family during a time of crisis by equipping them with the tools needed to honor your healthcare wishes.
