Requesting Personal Health Information

• If you would like electronic access to your PHI, you can sign up for our patient portal, YouLink at Hendrick.
• If you would like a paper copy of your PHI, print out and then complete and sign the Release of Information form and follow the instructions below.
  
  • Bring the form to Hendrick Medical Center ROI dept or
  • Fax the completed form to 325.670.6503 or
  • Mail the completed form to:

  • Abilene, TX 79601

No PHI will be released without picture ID with signature for confirmation of identity.
