• If a person has started on new medications and needs education about those medicines and evaluation of the effectiveness of the medicines.

• Post Hospitalization
• Discharge from a Community Skilled Unit
• Someone newly diagnosed with an illness
• Exacerbation of illness, such as CHF, Diabetes
