The new Texas Standardized Credentialing Application fulfills requirements of Senate Bill 544 (Acts 2001, 77th Leg., ch. 1369, §3, eff. Sept. 1, 2001), providing for the Texas Insurance Commissioner to adopt a standardized form for verification of physician credentials.
